Section 1

Who we are

NephroReach LLC is a South Carolina limited liability company that provides kidney health education to nephrology practices and, through those practices, to their patients. In this policy, "NephroReach", "we" and "us" mean NephroReach LLC. "You" means a visitor to this website or a user at a subscribing practice.

This policy explains what we collect, why, who we share it with, and what you can ask us to do about it.

Information we collect

Information you give us

  • Contact details you enter on this website, including your name, role, practice or organization name, work email address and mobile number.
  • Whether you checked the box consenting to receive text messages, and the date and time you checked it.
  • Anything you write to us in a form, an email, a text message or a support request.
  • Account details for users at a subscribing practice, including name, work email and role.

Information collected automatically

  • Standard web server information such as IP address, browser type, pages requested and referring page.
  • Analytics about how this website is used, so we can tell which pages are worth keeping.

Information a practice gives us about its patients

A subscribing practice may enter patient information in order to enroll a patient in a curriculum pathway. Where that information identifies a patient and relates to their health, it is protected health information and it is handled under Section 6 of this policy, not under the ordinary terms above.

Section 3

How we use information

  • To respond to a walkthrough request or a question you sent us.
  • To provide, support and administer the NephroReach platform for a subscribing practice.
  • To send account and implementation text messages, if and only if you opted in. See Section 4.
  • To send service email about your account, such as logins, billing and material changes to the platform.
  • To improve the curriculum and the product, using aggregated and de-identified usage data.
  • To meet our legal, tax and regulatory obligations.

We do not sell personal information, and we do not rent or trade contact lists.

Section 4

Text messaging and mobile information

If you check the consent box on our walkthrough request form, we will send text messages to the mobile number you provided. That form is the only way we collect consent for text messages. These messages are account and implementation messages for practice staff. We do not send marketing text messages and we do not text patients.

Mobile information is never shared

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All of the above categories exclude text messaging originator opt-in data and consent. This information will not be shared with any third parties.

Your mobile number and your consent record are used only to deliver the messages described in our Text Message Program page. We pass your number to our messaging carrier solely to transmit those messages to you. We do not pass it to anyone else, for any purpose.

You can stop the messages at any time by replying STOP. Message and data rates may apply. Message frequency varies.

Section 5

Who we share information with

We share information only with service providers who need it to run the business, and only for that purpose. They are bound by contract to protect it and may not use it for their own purposes. These include:

  • Our hosting and infrastructure providers.
  • Our messaging carrier, for delivering the text messages you opted in to.
  • Our email provider, for service and support email.
  • Our payment processor, for subscription billing. We do not store card numbers.

We may also disclose information where we are required to by law, or to establish or defend a legal claim. If NephroReach is ever acquired or merged, information may transfer as part of that transaction, and we will say so before it happens.

Subject to the exclusion stated in Section 4, which applies without exception.

Section 6

Protected health information

Where NephroReach processes protected health information on behalf of a subscribing practice, it does so as a business associate of that practice under a written business associate agreement, in accordance with HIPAA. The practice remains the covered entity and controls that information.

We use protected health information only to deliver the service to the practice and as the business associate agreement permits. If you are a patient and you want to know what your practice holds about you or how it is used, contact the practice directly. They are the right door, and they can reach us if they need to.

Section 7

Retention

We keep contact and account information for as long as the relationship is active, and afterwards only as long as we need it for legal, tax or dispute purposes. Consent records for text messaging are kept for as long as we have your number plus the period our carrier obligations require, because the record of consent is what proves we were allowed to text you.

Patient information held on behalf of a practice is retained and returned or destroyed as the business associate agreement with that practice provides.

Section 8

Security

We use encryption in transit, access controls limited to the people who need them, and reputable infrastructure providers. No system is perfect and we will not claim otherwise. If a breach affects your information, we will notify you and any regulator that has to be told, within the time the law allows.

Section 9

Your choices

  • Text messages. Reply STOP at any time. You can also email or call us and we will remove you.
  • Email. Use the unsubscribe link in any non essential email, or ask us.
  • Access, correction and deletion. Email hello@nephroreach.com and tell us what you want. We will respond within 30 days. If you are a patient, contact your practice, as described in Section 6.
  • Analytics. Most browsers let you block cookies. The site works without them.
Section 10

Children

This website and the NephroReach platform are intended for use by adults. We do not knowingly collect information directly from anyone under 13. Where a practice enrolls a minor patient, that is done by the practice under its own consent and authorization processes.

Section 11

Changes to this policy

If we change this policy we will update the effective date at the top of the page. If the change is material, we will tell account holders by email before it takes effect.
